Short version: a visit request goes to the front desk of one small practice in Atlanta, it is used to call you back, and it is not sold or shared with advertisers.

What this website collects
Only what you type into the visit request form: the name the appointment is for, a phone number, an optional email address, the visit and format you selected, a preferred day and time window, an optional insurance carrier or claim number, and whatever you write in the description box.
The site sets no advertising cookies, runs no third-party trackers, and does not build a profile of your browsing. Fonts are requested from Google Fonts, which means Google receives the request for the font file itself.
How that information is used
- To call or text you back about the appointment you asked for
- To verify your benefits with your carrier before the first visit
- To open a patient chart if you become a patient
- To respond to billing, records or claim questions you raise
It is not used for marketing lists, is not sold, and is not passed to anyone outside the practice except a carrier or claim adjuster you have asked us to bill.
Protected health information
Once you are a patient, your chart is protected health information under HIPAA and under Georgia law, and it is handled under the practice’s Notice of Privacy Practices — a paper copy is given at the first visit and another is available at the desk any time. Records are released only with your written authorization, or where the law requires it.
Please keep clinical detail off email and web forms where you can. A short line about the complaint is useful for triage; the full history belongs in the exam room.
Retention and access
Visit requests that never become appointments are cleared within twelve months. Patient records are retained for the period Georgia requires for chiropractic practices. To see, correct or request a copy of your record, call the desk or write to the address above and we will confirm the request in writing.

Accessibility
The site aims for WCAG 2.1 AA: keyboard-reachable controls, visible focus, real text over images of text, and contrast that holds up. The building entry is level with no stairs from the lot. If something here is hard to use, or you need help completing a request by phone, call the desk and a person will do it with you.
